In addition, information on the fact sheet (PDF) or funeral-related form. 
Could you add the name of the sacristan entered when creating the Funeral form in addition to the name of the accompanying family.

This is the PDF sheet sent to the celebrant and to the carers of grieving families in order to facilitate the organization of the celebration (since the parish includes several sacristans)
